At Crossroad Home Health and Hospice whether returning home from the hospital, a skilled nursing facility, or a doctor's visit, their in-home team of Nurses, Therapists, Medical Social Workers, and Home Health Aides help our patients achieve independence in their own community. Their Palliative Care Bridge Team consists of trained and certified Professionals who work with patients with chronic illnesses, acute illnesses, high hospitalizations, poor Prognosis, or need pain management.
